If a plug wasn't tightened properly when it was replaced, it could have just loosened up over time until it finally popped out. If that's what happened, all you need to do is screw it back in.

Do use a gapping tool to gently open the ground electrode air gap. To reduce the gap, hold the spark plug straight and gently tap the ground electrode against a hard surface.
